Your care team will assess your health data and communicate with you via the Capio Hemma app.

Who can use the service?

Capio Hemma is designed for individuals, 18 years or older, who want to gain control of their health with the following diagnosis: chronic heart failure, COPD, type 2 diabetes and / or high blood pressure. You can get access to Capio Hemma via referral by a Capio primary care clinic or by starting a private subscription.
